Renewal of concessional Telephone facility to retired employees / spouses of DOT / BSNL

It is hereby notified for information of the retired employees / spouses of the deceased employees availing of concessional telephone facilities that  :

A.   Life Certificate must be furnished by such retired employees or his/her spouse after every one
       year to the concerned Telephone Authority.

B.   In view of the above, the concern persons are required to furnish the following documents to
      the DE (CR), calcutta Telephones, Telephone Bhavan, 7th Floor, Kolkata - 700001 from
      01.09.2006 to 31.10.2006 at Receiving Section / Life Certificate Box placed at Ground Floor,
      Telephone Bhawan during office hours in all working days  :-

      •    A declaration by such persons (availing concessional telephone facility) duly authenticated
             in the proforma, available in this website as well as in the office of Sr. SDE/(CR-I & II),
             Calcutta Telephones, Room No. 727, 7th floor, Telephone Bhawan during office hours in all
             working days from 01.09.2006 to 31.10.2006.

      •    Photocopy of the last Telephone billand Eligibility Certificate.

Note  :
      1.     Non-compliance of the formalities will cause discontinuance of the concenssional
              telephone facility.

      2.     In case of any difficulty, please contact DE (CR) or SDE (CR-I & II), Calcutta Telephones
              personally with PPO/Bank Pass Book, Telephone Bill and Eligibility Certificate for
              necessary action.

      3.      The above instruction is subject to modification as may be notified from time to time.
